ERP Therapy

Breaking the Loop, One Step at a Time.

Woodbury | New Haven, ct

Facing What Fear Tells You to Avoid

ERP (Exposure and Response Prevention) is the most extensively researched treatment for OCD and anxiety disorders. It works by helping you confront what fear demands you avoid, while resisting the compulsive responses that temporarily ease distress but ultimately reinforce the cycle.

OCD is not about cleanliness or order. It is about the trap of intrusive thoughts, unbearable uncertainty, and compulsive responses that feel impossible to resist. Whether those compulsions are visible rituals, silent reviewing, or reassurance-seeking, the mechanism is the same: the brain gets caught in a loop, and every compulsion tightens it. ERP interrupts that loop at its source.

At Higher Ground, ERP is practiced by Will Culpepper, LMFT, a clinician with the ability to see a client's true resilience beneath the behaviors that brought them in. He reads not just the OCD, but the person carrying it. He knows when to push and how hard, helping clients discover a capacity they didn't know they had. Clients trust him quickly. And that trust becomes the ground on which the hardest work becomes possible.

You don't have to be fearless. You just have to take one step.

Who Is ERP Designed For?

ERP is designed for individuals living with OCD and anxiety disorders where intrusive thoughts, compulsions, avoidance, or panic have begun to organize their daily life, relationships, and sense of what is possible.

  • Adults with OCD in any form: contamination, harm, scrupulosity, symmetry, or “pure O”

  • Individuals with anxiety disorders including panic disorder, generalized anxiety, or social anxiety

  • People with specific phobias or avoidance patterns that have narrowed their lives

  • Those whose compulsions are covert: mental reviewing, reassurance-seeking, or internal checking

  • Individuals with health anxiety or intrusive thoughts that feel impossible to dismiss

  • People who have tried talk therapy without finding lasting relief from OCD symptoms

  • Anyone whose daily routines have been shaped by what fear allows or forbids

Key Benefits

A Scientifically Proven Method

ERP is the gold standard for OCD, endorsed by the American Psychological Association and the International OCD Foundation as the most effective available treatment.

Built for What Other Therapy Misses

Many people with OCD have spent years in talk therapy without meaningful relief. ERP targets the mechanism of OCD directly, not the content of intrusive thoughts.

Structure That Builds Real Confidence

ERP doesn’t teach you to ignore fear or think positively. It teaches you that you can face what the mind has labeled dangerous, resist the compulsion, and survive. With each exposure, the evidence builds and the brain’s threat prediction begins to shift.

The ERP Process at Higher Ground

Assessment & Hierarchy Building

Before any exposure begins, we carefully map the landscape of your OCD or anxiety. We identify specific triggers, intrusive thoughts, and compulsive responses, then build a graduated hierarchy from least to most distressing. The work never moves faster than you understand and are prepared to try.

Graduated Exposure Practice

In ERP sessions, we move through your hierarchy step by step, confronting feared situations, thoughts, or sensations while resisting the compulsive response. This is where the central learning happens: your nervous system discovers that discomfort passes, and that you can tolerate uncertainty.

Response Prevention & Habituation

Resisting the compulsion is the core mechanism of ERP. Over time, as you repeatedly face feared situations without performing the ritual, anxiety naturally reduces. What once felt catastrophic begins to feel manageable. The loop loosens. The brain revises its predictions.

Integration & Relapse Prevention

As the hierarchy clears, we work on maintaining gains, identifying early warning signs of relapse, and building confidence in your ability to handle difficult thoughts without needing a ritual to restore a sense of safety.

Frequently Asked Questions

What is ERP and how is it different from regular therapy?

ERP is different from traditional talk therapy in that it doesn’t primarily focus on understanding thoughts. It focuses on deliberately practicing facing feared situations or thoughts while resisting compulsive responses. Over time, this teaches the brain that discomfort is tolerable and that uncertainty doesn’t require a ritual to manage.

Does ERP mean I have to do things that terrify me?

Yes, but in a graduated, supported way. We never begin at the top of your fear hierarchy. We start with situations that feel manageable and build systematically, at a pace your nervous system can handle. The work is challenging by design, but never reckless or unsupported.

How many sessions does ERP typically require?

Most clients see meaningful improvement within 12–20 sessions, though the pace depends on severity, complexity, and whether intensive or weekly formats are used. Some clients choose periodic maintenance sessions once primary goals are achieved.

Does ERP work for all forms of OCD?

ERP has strong evidence across OCD subtypes, including contamination OCD, harm OCD, scrupulosity, pure O, health anxiety, and symmetry or order concerns. Specific exposures are tailored to your exact presentation and the unique way your OCD manifests.

What if my compulsions are internal, like mental reviewing or reassurance-seeking?

Internal or covert compulsions are common and are addressed directly in ERP. Response prevention applies equally to mental rituals. We identify them, map them carefully, and build strategies that reduce their grip over time, the same way we address visible behaviors.

Do I need to take medication to do ERP?

Medication is not required, though some clients find it makes ERP more accessible by reducing baseline anxiety. We coordinate with prescribers when relevant, but ERP alone is highly effective without pharmaceutical support.
